Requests for an excerpt or a copy of personal and payroll files should be sent to the above correspondence address.

In the body of the letter, please include:

  • name and surname (optional: maiden name),
  • date of birth,
  • full correspondence address,
  • phone contact number,
  • information on what documentation the request for an excerpt or copy concerns (from which company, from what period).

If an inquiry is received, employees of the CDA ArchiDoc Customer Service Department check the status of the documentation, its feasibility and evaluate the order within seven days. The cost of making an excerpt or copy is covered by the ordering party after receiving the parcel with the ordered documents.

The amount of fees for the services provided is regulated by an internal order of the President of the Management Board of ArchiDoc SA, taking into account the guidelines contained in the Ordinance of the Minister of Culture of February 10, 2005 on the determination of the maximum amount of fees for the preparation of an excerpt or copy of documentation with a period of temporary storage.

Should any potential claims be filed – the data shall be processed for the purpose of establishment, enforcement of or defense against such claims. The User’s personal data shall be processed solely in order to enable ArchiDoc S.A. to respond to the User’s inquiry. Provision of the personal data is voluntary, but failure to provide them shall prevent ArchiDoc from responding to the inquiry submitted. The User’s personal data shall be stored until their consent is withdrawn and until the expiry of the period of limitation for any potential claims. Your data shall not be transferred to any third parties other than entities authorized to process data in accordance with provisions of the law. The Controller does not intend to transfer the User’s personal data to third countries or international organizations, with the exception of services provided by Google Analytics and Google AdWords. The User may request that the Controller provide access to their data, rectify, transfer or erase them, or restrict the processing of the User’s data. The User shall have the right to withdraw their consent at any time by submitting a relevant declaration to: iod@archidoc.pl and, simultaneously, to archidoc@archidoc.pl. Withdrawal of the consent shall not affect the lawfulness of processing performed based on the consent prior to its withdrawal. The User shall have the right to lodge a complaint to the President of the Office for Personal Data Protection in connection with the processing of their personal data by the Controller. The Controller shall make no automated decisions with regard to the User, including by means of profiling, based on the User’s personal data.
